It not only suggests the recipient of the insult might have sexual feelings for their parents but also suggests their parents are extremely unusual and of an ambiguous gender. This term can be altered to read, lick your fathers cunt: this means the exact same thing. Both of these terms imply gender confusion and an overarching sense of confusion in general. A further implication is that the recipient of this term engages in incest and they themselves are inbred and therefore intellectually deficient.
