“Case of the cold pecker” is usually a joking/slang way of saying someone suddenly got nervous, lost confidence, or backed out of something — kind of like saying they got “cold feet.”
Jake: “You were gonna ask her to prom yesterday, what happened?”

Mike: “Man… I got a case of the cold pecker and bailed at the last second.”
