A post mortem review of a failed organization, government, or public/ private institution to determine the likely factors that led to its downfall or demise.
An organizational autopsy of companies like Enron, Block Buster, or Toys R Us will provide insight into what caused their downfall so we can avoid the same fate.

Organizational Feedback Index refers to the scientific concept of measuring employee empowerment numerically. It further obtains perception of the organization towards feedback through a well defined algorithm.

Organizational Constipation 

When an organization cannot get anything accomplished in a reasonable amount of time; A lack of quick turn around.

When an organization, primarily a bureaucracy, cannot get shit done because of back log and/or inefficiency.

organizational terrorist 

An organizational terrorist is a person or subordinate who asks tough questions to their boss and brings out the greater greatness of the individuals around them, but the current chief officers don’t like being questioned or challenged so they label these people and try to segregate them to deter promotion within the organization and slander the person.
There was a Captain that is on the rise to become a chief officer, he used a common sense approach and all his employees loved him. When he would challenge the union thugs or chief officers with new ideas, challenging questions or question the chief officer decisions they hated his common sense approach. When a deputy chief was training captain candidates he referred to the beloved Captain as an ORGANIZATIONAL TERRORIST because that captain was not a "yes" man, when in fact that chief and his wine club are the organizational terrorist.

The Unification Organisation for Collective Ascension made me pay my wages into their treasurer and I feel like I can't escape.

Organizational Stranger

When half of a department, team, organization, etc. has only a vague clue what is going on in the other half of the department. This is especially relevant to group projects.
Looks like we've just experienced an organizational stranger.
