Someone who is of an extreme Liberal point of view and who has elevated COVID-19 prevention or mitigation to the point of a religious persuasion as a measure of political correctness, like they have become a zealot and judge and shame others for their “lesser” measures and difference of opinion. A Masker uses COVID-19 prevention to make a display of their extreme Liberal persuasion and self-defined political correctness, especially for others to see. Reference COVIDIAN as a similar term.
“Look at those two MASKERS! They just commented to that guy because he pulled his mask down in public to take a drink of coffee!” Or, “I wash my hands frequently and wear a mask, but I'm not going all MASKER and posting on social media about how we should be keeping the mask on even while driving solo!" A decent human being that cares about people other than themself during a pandemic, or when they are generally sick, making sure to wear a mask for everyone else's safety.

This person wears Covid-19 mask while driving alone in their car. An ultra masker will also wear a mask while walking alone on the sidewalk, riding a bicycle, jogging or going on long nature hikes. An ultra masker will do all these things either before or after they have been fully vaccinated.
I was stuck in traffic on 285 yesterday and saw an ultra masker alone in her car jamming to Snoop Dogg.
