The "Kurt Cobain'd" joke was originally created by a YouTube gamer, while making audio commentary for a playthrough of the popular role-playing game Fallout 3.
Getting "Kurt Cobain'd" pretty much equals to getting shot in the face by a shotgun. (also referred as "Shotgun Rain to the face")
It is also a reference to the death of the Nirvana band's lead singer, Kurt Cobain, who committed suicide by shooting himself in the head with a shotgun.
