This term usually said when something is "out of order". Usually when speaking of an automated machine.

Triple-o sauce

the sauce served at whitespot. The makeing of it consists of milking a magical gnomes gentalia. when it is gathered it is then mixed with eggs and side of hash. Then it is baked to a milky white coulour and served with a side of magical mayo from a unicorns horn.
