When you envision yourself as the main character of the movie version of your life, highlighting the parts of yourself that would make you unique or likable. This syndrome focuses on the self more than any other person, much how movies or television have a story primarily based on one character.
Cindy has main character syndrome and am constantly imagining herself as the title role in the movie version of her life: THE UNTOLD CINDY STORY.
Cameron Diaz has main character syndrome in The Holiday when she keeps envisioning movie trailer versions of her life, with herself as the title role.

Another form of narcissism in which the person (usually popular) believes they are the main character in their life, and in everyone else’s too. they usually believe that their opinions are correct and important. And usually get involved in other peoples lives and try to influence said people into making choices and outcomes that revolve around them to further push the delusion that they are more important and are the main character of life
“Kimmy was sitting with the chess club during lunch today and taking pictures with them”
“What ? Wasn’t she making fun of them last week calling them nerds and virgins ?”
“ I mean, what’d you expect? She has main character syndrome, she thinks it’ll make her look better on social media if she’s hangs with the nerd crowd, she really doesn’t plan of being their friend”

A somewhat naive term referring to "When someone thinks they are the main character of their life."
It is naive because everyone IS the main character of their life, everyone is the main character of their own story. Believing you are the main character of the whole world, or all of time is called narcissism, and this term is basically a way to conflate things like focusing on yourself or your ambitions with narcissism.
I would not recommend using it unironically, or towards this definition at all if you have any self respect.

Hym “Main Character Syndrome. The idea that you are the protagonist of the story or that you should be the main character. Is that how you’re acting? And that’s what I mean about ‘Only telling half the story.’ Because you’re not just the person who nobly endures the suffering at the hands of the mob. You’re not just the person who’s called forth to adventure. You’re the mob that demands God’s angels be given to them. You’re the horde that rends Jesus asunder. The tyrant the needs to be taught a lesson by having his kid slaughtered. I mean... are your REALLY the Abraham? Are you REALLY emulating Christ? Or are you just acting?”
