Taken from the term “second string” in an athletic competition situation. In football, if the star quarterback gets injured during a play, the second string is called off the bench to replace him/her. A “Second String Friend” is essentially benched until needed, if ever.

As for what it is, it’s a lung disease caused by inhaling dust and ash from volcanoes, so it’s a variant of silicosis and pneumoconiosis. It is said to be a made-up word intended simply to be the longest word in the English language, but it isn’t, that title goes to the chemical name for Titin which is a type of protein. In English this word has 189,819 letters and would take three-and-a-half hours to pronounce.
