Phrase spoken to a person who walks in on a conversation that would sound wrong if they heard it.
Example 1:
Person 1 - I wouldn't worry about it. The computer says the average size for a 15 year old is 6.579 inches.
Person 2 - Yeah.

Person 1 - How you doing? Doing good?


Example 2:
Person 1 - But you have to pay a lot of money for those.
Person 2 - I agree totally. I would never use those.
Person 1 - See, sometimes, I take my toy lightsaber and...

Somewhat archaic greeting. Was popular in England in polite society before the 50s.

The phrase is somthing of a non-sequiter - if read as written there's only half a verb, because one usualy does somthing rather than just does. The question could be construed as 'doing' your girlfriend or wife, but the intent is not to open a discussion on sexual acts, but rather to greet.
Jack: How do you do?
Jill: I am well Jack, and yourself?
