1. This is a term used by the people of the southern United States particularly near the Gulf of Mexico to express to someone that they are an idiot without saying such harsh words.
2. "You are an idiot but I like you and care about you so I dont want to hurt your feelings."

Little Billy: I am 6 years old (only holds up 4 fingers)
Scarlet: Oh honey, bless your heart, but that's only 4 fingers.
Little Billy tries again: I am 6 years old (this time holds up the same 4 fingers and 4 more on the other hand)
Scarlet: Child....Bless you and your momma's heart.

A southern way to cuss out the ignorant.
You: "OH my gosh, I just failed my midterm after I spent all my time with 'X'!"
Any southerner: "Bless your heart."
Ex 2
You: "wow, can't believe there is a whole other culture down here!"
Some southerner: "Bless. Your. Heart."
When with friends, it is sometimes said in a teasing manner while still calling you out for being dumb

As a southerner that has been in war torn countries and seen children suffering, "Bless your heart" has it's true meaning. "Bless your heart" is a is a statement of empathy. Most Southerners shed a tear when they speak those words. True, at times they are offered for fools, such as those who can't understand the true meaning of the words. More's the pity! Bless your heart!
I came upon a child that was scared and burned. I cried, saying, "Bless your heart" as I pulled out my MEDKIT and prayed that I could ease her suffering!
