Some one who has a small head for their body size. Also can be characterised by a small jaw and/or large ears. Obviously, some one with a small head, receeding jaw and large ears can be described as "piphead extremis".
While watching the TV we noticed that the presenter was a Piphead. The more we looked the more obvious it became.

A popular meme from the show “Spongebob Squarepants”. In an episode of “Spongebob Squarepants” called “Survival of the idiots”, Spongebob and his best friend Patrick act as two famous outlaws, pinhead Larry, and dirty Dan. Pinhead, is used as an insult, and Patrick doesn’t like that.
-Spongebob- -western accent- “Alright pinhead, your time is up!”, calls him pinhead cause he has a pinhead -Patrick- -western accent- “who ya callin pinhead”, cause he takes as insult. Pinhead is a rude compliment, said by spongebob. Pinhead Larry, defined as rude and Un intelligent
