To evakluate means to evacuate a premises while in the process of evaluating it, normally under extreme circumstances. (Otherwise... why would you do both?)

The health inspector had to quickly evakluate the restaurant because of the fire that began in the kitchen.

To re-evaluate is a concious decision made by an individual, who has possibly hit rock bottom, to develop a plan in order to get their life back on track in an attempt to climb their way back up the social ladder.
