The case of Shawn Hornbeck is a recent case that got a lot of media attention in which Sylvia Browne told the parents that their son was dead and exactly described what happened and where he could be found. The police found the boy alive and well and Sylvia's story was shown to be completely false.

The most vague psychic since Ms. Cleo. Can be found on many episodes of Montel answering questions like "Is my dead dog mad at me for not saving him from the housefire" with a response such as "No. He or she could or could not be mad at you. That's because he or she is in fact either not living or no longer in your life"
Did you see that Sylvia Brown on Montel? She guessed that the dead relative was a man! Then again, she had a 50/50 chance of getting that right. But still, it's amazing!!
